In the early hours of December 14th, 2015, Chicago firefighters were called to an industrial building fire on the 9200 block of South Baltimore Avenue. The fire broke out in a three-story structure that was under construction, and it was reported that there were numerous open holes in the floors due to ongoing work. As firefighters worked to control the blaze, a distress call known as a Mayday Alert was issued when a firefighter fell through one of these holes.

The firefighter, identified later as Dan Capuano, a 15-year veteran of the Chicago Fire Department, was conducting a search on the second floor when he fell into an open elevator shaft leading to the basement. Emergency medical services were immediately deployed, and despite their efforts, including performing CPR at the scene, the firefighter was pronounced dead upon arrival at Christ Hospital.
Chicago Fire Commissioner Jose Santiago confirmed that Capuano was part of Tower Ladder 34 and had fallen into the shaft while actively engaged in firefighting operations. The fire, which originated on the second floor, was brought under control roughly an hour later, around 3:45 AM.
